The surgery is required to invite patients for blood pressure checks for various conditions including CHD, Hypertension, and Stroke/TIA.

Patients who choose to decline blood pressure checks via this form will have their medical record updated to show this.

Patients who complete the form should not receive another invite until the next financial year. Patients can then choose to decline again if they so choose.

The surgery recommends having regular blood pressure checks for long-term medical conditions. This enables us to provide patients with the best possible care.
